
Family Nurse Partnership
Description of work
The Family Nurse Partnership is a programme of home visits that may be offered to first time parents aged 19 and under. it is intended to help young, first time mums understand more about their baby's health, well being and development. The visits statrt in the early pregnanacy and will take place until your babay is two years old.
During the visits you will have lots of time to talk about your baby, your family and yourself. we can help you to understand your baby's development. if you wish, your Family Nurse can support you to stop smoking. we can help you to make plans for your future (educationor employment).
The home visits will be from specially trained nurses caleed Family Nurses. The Family Nurses only work with young families and your partner and other members of your family will be encouraged to join in the visits if you wish. You will still see your midwife who will carry out your midwifery care as normal. Aftre your baby is born, your Family Nurse will deliver the Universal Child Health promotion programme until your child reaches their 2nd birthday. This is done by a Health Visitor for mothers not on the FNP programme. When your child reaches 2 years of age your Family Nurse weill handover your care to a Health Visitor.
